You can also do it this way without recursion. data and descr are of course just mock ups for your tables, you would not need any WITH clauses. This method uses the steps (1) break up the sentences into words, (2) outer join using those words to your description table, replacing any matches with the description values, (3) reassemble the words back into sentences using LISTAGG.

You could use PIVOT to get the var values from rows into columns (geting all vars in the same row with text) and then do multiple replaces depending on number of var values:

I have a table with parent/child ids, and I'm trying to get a full list of all levels of parents AND children for a given id.
Basically, for a given id, go all the way down and all the way up the hierarchy.
I've tried connect by, but maybe a recursive CTE would be better?

Here's my attempt. It seems kind of hacky with the full_hier being a concatenation + substring of the child & parent paths. Plus, I'm getting additional results that I'm not sure how to filter out (Ex: def > abc is returned though I don't want it as it's captured in 789 > def > abc).

If I understand correctly, given any id (as an input - below I use a bind variable in the query), you need to find all its leaf descendants, and then for each such leaf, show the full path from leaf to the root of the hierarchy.
One way to do that is to traverse the hierarchy twice: first start from the given id and find all its leaf descendants, then traverse in the opposite direction to find all the "full paths".
While this may look (marginally) more elegant, it will be significantly less efficient. The better approach is along the lines you were trying already.
Below I use the with clause (and give column names in the subquery declaration - that is only supported since Oracle 11.2, if your version is 11.1 you will need to move the aliases into the select clauses as you were doing in your attempt).

Here's a method that first descends down the tree to get the root parent(s) from the chosen one.
Then uses those roots to climb back up.
The resulting paths are then filtered on the chosen child.
And what remains are those where the final child isn't a parent.

How can we write a update on the table so that it only updates the corresponding value.

Don't store values in delimited strings.

It is running fine in SQL developer but when executing it as a procedure, I am getting INVALID NUMBER ERROR (ORA-01722: invalid number) at

The error is pretty clear. You're comparing a number to another type of value.
Example:
SELECT 'x'
FROM DUAL
WHERE 1 IN (SELECT 'a'
FROM DUAL)
This means that IP_LNT_ID, LNT_ID, LPN_LNT_ID and LPI_ID have to be NUMBER. And LPI_DATE_ADDED and LPI_DATE_ADDED should both be date or timestamp.
If this is not possible you could compare everything as char:

But this should be avoided on any cost. Unfortunately some times we have to cheat a little from time to time to work with our existing infrasructure ;-)

Have the same data type or cast/convert one or other so that they have the same data type.

I'm creating a report with SQL server 2012 and Report Builder which must show the total number of Risks at a high, medium and low level for each Parent Element.
Each Element contains a number of Risks which are rated at a certain level. I need the total for the Parent Elements. The total will include the number of all the Child Elements and also the number the Element itself may have.
I am using CTEs in my query- the code I have attached isn't working (there are no errors - it's just displaying the incorrect results) and I'm not sure that my logic is correct??
