I am trying add a photo in the SQL Management Studio 2018. But it tells to use bulk load statement. On the Stackoverflow I saw some steps that may help me. It was recommended to go to the login->security->properties and activate 'bulkadmin'. This operation failed saying that it is not enough permission.
If you are trying to grant the bulkadmin Server Role to a Login, the login you are using to make this change will need to either have CONTROL permissions on the Login, or the ALTER ANY LOGIN permission on the SQL Server itself.
The easiest solution if possible would be for your own Login to be granted the sysadmin Server Role by another Login with the necessary permissions to do this.

I am working on a project and I have access to SQL Server as external user with limited privileges.
When I want to create a login for example with this command, I get permission denied:
CREATE LOGIN [login] WITH PASSWORD=N'test', DEFAULT_DATABASE=[master],

However when I try to create a login with this command I can make it and also I have privileges now to enable xp_cmd shell as well:

EXECUTE('string sql statement') AT "hostname\domain" == the 'string sql statement' is a pass-through command executed at the linked server "hostname/domain".
Has someone created a loop-back linked server (a linked server that points to the sql instance itself)?
Linked servers have their own security configuration/settings. If the security of the linked server is configured (for any login) to be made under the security context of a privileged login(eg. sa) then exec('') at linkedserver will be executed with way more/elevated permissions (than expected). This is a major security issue/hole.
Check the security of the linked server and change accordingly (and do you really need a loopback linked server?)
I have been able to set up an active directory login for my Azure database.
Now I want to add additional active directory logins and set their permissions
I understand that I need to use Management Studio to do this and have installed SSMS v17.8.1

After reading Conor's answer I was able to create the user
CREATE USER [jobuser#mydomain] FROM EXTERNAL PROVIDER WITH DEFAULT_SCHEMA=[dbo]
However if I try to login using MSSMS with Active Directory - Password authentication I get
Login failed for user 'NT AUTHORITY\ANONYMOUS LOGON' (MIcrosoft SQL Server, Error 18456)

That particular page only shows an example using a SQL login/user, so I can understand why you are confused.

Specifically, you want to create a user using the AS EXTERNAL PROVIDER example. This tells SQL Azure (and SQL DW) to use Azure Active Directory instead of creating a SQL user. Please note that this does not (as of today) work in SQL Server. (SQL Server supports integrated authentication against a local Active Directory, but it just does not currently support the Azure Active Directory service).
Also, there is no specific requirement that you use SSMS to do this step. You can do it within SSMS, SQL Operation Studio, through one of the SQL client drivers in a program you write (or some third party program that can invoke SQL over them), sqlcmd, etc. You don't even need to have the latest SSMS to make things work (though it is recommend to get the best experience against SQL Azure including syntax highlighting).
Please give that a try and see if it gets you unblocked.

I have searched the internet and all posts and articles say that I need to log in as administrator or SA. I am logged in as administrator and still can not gain permission to create a database.
How do I assign permission to the SA account to create a new database?
Problem here is that you are logged in with the local Windows Administrator account. You have two logins for your SQL Server instance as can be seen in your Image #2. One is SA (SQL Admin). The other is BUILTIN\USERS. That is a default group which every user that logs into the machine will be a part of. But unless you give that group permissions, or create other logins (I recommend this), you will be very limited to what you can do.
As you have found, that user group cannot modify permissions, especially for the SA user. The SA user is special, you cannot modify SA permissions at all. This is why you are receiving errors (partly because you don't have permissions to change them with the user you are logged in as, and partly because SA permissions cannot be modified).
The resolution to your issue is to login with the SA account. You will need to choose "SQL Login" at the login screen, as opposed to Windows login. SSMS will prompt you for a password. This password was configured by the person who installed the SQL instance. If the person who installed SQL Server did not specify a password, it will be most likely be a blank password or "Password123".
Once you are logged into the SA account, you can create new SQL or Windows Authentication logins and and provision them permissions as needed.
I am unable to grant access to a newly restored database using sp_grantdbaccess. I am trying to do this via dynamic sql like below.

Based on the error message it looks like you are trying to grant access to IIS APPPOOL\myApp using the security context IIS APPPOOL\myApp, but IIS APPPOOL\myApp doesn't have the access rights to the db to grant access rights to itself. I think this is akin to me trying to grant myself access to my neighbors house, but I don't have any authority to do so.
Depending on what type of security policies you are working under, I would either run the app pool under an account that has the greater privileges through integrated auth (but this might negate your need to grant privileges), or execute these SQL statements using a local SQL account with the necessary privileges to the database. In these cases, it would be like asking my neighbor (elevated access) to let me (IIS APPPOOL\myApp) in.
If you think it should work because IIS APPPOOL\myApp had access to the DB prior to being backed up, could this be a problem with IIS APPPOOL\myApp being an orphaned user? See http://msdn.microsoft.com/en-us/library/ms175475.aspx But with this case, you still might run into the above scenario trying to fix the orphaned user.
I'm new to SQL Server 2008. I just installed SQL Server Express. I'm having trouble creating a new database, and I think I don't have permission.

I resolved my problem with the following steps:
Set the instance of the SQL Service to single-user mode:
Open SQL Server Configuration Manager. Double click SQL Server Services.
Stop all SQL Server services
Right click SQL service and click Properties, in the Advanced tab, look for 'Startup Parameters'
Insert '-m;' at the beginning of the Startup Parameters value
Start the SQL service
Open SQL Server Management Studio and login with Windows authentication, you can now add user or change password of different users.

Try logging in with the sa account and grant permissions to your Windows account.
If you do not know the sa password use sqlcmd and execute the following commands:
Use Master
Go
ALTER LOGIN [sa] WITH PASSWORD=N'NewPassword'
Go
Login with the sa account and GRANT permission to the account.
