I am using xmllint to clean up my .xml file but for some reason the output adds <?xml version="1.0"?> to the first line. Is there a flag to have this be removed?
Both of the following seem to avoid insertion of <?xml version="1.0"?>:
xmllint --exc-c14n data.xml
xmllint --c14n data.xml

Anyway, when I build the book (I'm using personal edition of XMLMind), I don't see anything. In the PDF the example is printed, but instead of the source code inside the main.cpp I see

What I'm doing wrong? How can I include the source code from the file?
I've reproduced your situation in a different ways and got things working in a both cases.
You didn't mention the exact steps you make to build a book using XMLMind XML Editor (XXE for short), but the following steps certainly worked well.
None 1: I've used XXE Professional Edition, but as I know the difference between versions is only in putting random character within text if generating PDF from Personal Edition.
Note 2: XXE says that <description>Book chapters</description> element is not allowed within you context. Despite that fact, the assembly can be generated successfully.
1. Getting things working within XXE
Open an assembly file in XXE.
Select Convert > Convert document > Convert to PDF.
Select a path to save pdf file and press OK button.
2. Getting things working manually in command line
Use your exact files and DIR structure (I included my own .c file sample for my own test).
Use XXE assembly processor: http://www.xmlmind.com/xmleditor/assembly.shtml - it has the same codebase that XXE used for working with assemblies from XXE GUI.
Get docbook book file from assembly:assembly-1_0_2_01/bin/assembly -v assembly.xml docbook_book.xml In my test I've got the .c sample file included - that proves that assembly utility works well with XInclude.
Get .pdf manually: fop -c <config_file> -xsl <path_to_docbook_ns_stylesheets>/fo/docbook.xsl -xml docbook_book.xml -pdf docbook_book.pdf
I've got the final result with .c sample included in both cases.
I am trying to process a large number of xml files (maven poms) using xmllint --xpath. With some trial and error I figured out that it does not work as expected due to the bad default namespace declaration in these files, which is as follows:

A simple command fails as follows:
$ echo $(xmllint --xpath '/project/modelVersion/text()' pom.xml )
XPath set is empty
If I get rid of the xmlns attribute, replacing the root element as follows:
<project xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance"
xsi:schemaLocation="http://maven.apache.org/POM/4.0.0 http://maven.apache.org/maven-v4_0_0.xsd">
The previous command gives the expected output:

4.0.0
Changing hundreds of pom files is not an option, especially since maven itself does not complain.
Is there a way for the xmllint to process the file with the bad xmlns?

Thanks to Damien I was able to make some progress:

But this does not quite do what I need. My follow up questions are as follows:
Is there a way to print only the text? I would like the output to contain on 4.0.0 in the above example
It seems the output gets truncated after about 30 characters. Is it possible to get complete output? This does not happen with xmllint --xpath

if you have funky formatting (like, the xmlns attributes are on their own lines), run it through the formatter first:
cat pom.xml | xmllint --format - | sed '2 s/xmlns=".*"//g' | xmllint --xpath '/project/modelVersion' -
xmllint --xpath "/*[local-name() = 'project']/*[local-name() = 'parent']/*[local-name() = 'version']/text()" pom.xml
For a top level pom.xml:
xmllint --xpath "/*[local-name() = 'project']/*[local-name() = 'version']/text()" pom.xml
It ain't real pretty, but it avoids formatting assumptions and/or re-formatting the input pom.xml file.
If you need to strip off the "-SNAPSHOT" for some reason, pipe the result of the above through | sed -e "s|-SNAPSHOT||".

Tried creating the file using following code but got the error message during installtion that file doesn't exists.

Best method to do this is first you copy a sample xml file as below.

When the staging build runs on Octopus this is what's in the log:
DEBUG [XML Transformation] The expected namespace http://schemas.microsoft.com/XML-Document-Transform was not found in the transform file.
Have tried to remove the other xmlns attributes to no avail.
Said namespace is clearly on the transform file as an attribute in the configuration node. We have numerous other such files and transformations are applied fine. The Build Action for all transform files is set to Content.
Not sure if we can make Octopus log such messages as errors instead of Debug though.
Has anyone else had a similar problem?

It looks like it might be trying to apply your forms.config as a transformation file instead of a target.
What do you have specified in the deployment step's Additional Transforms? Did you accidentally include forms.config itself?
I'm using MSBuild, FLAC and LAME to manage my MP3 collection, but it fails when it encounters a file with a non-ASCII character in the filename.
The following, cut down, project file shows the problem:

As you can see, something is munging the "™" and converting it to "T".
But what? And how can I fix this?
The built-in MSBuild Exec task writes the given command to a batch file and then runs it with cmd /c /q. Unfortunately, it writes the batch file with the OEM code page.
This means that Unicode filenames are mangled.
You'll have to write your own Exec (or Pipe, or even ConvertMusic) task instead.
